Greenwald Family Dentistry is excited to offer a comprehensive and cost savings plan for you. Our goal is to help you and your family keep a healthy smile even when you don't have dental insurance.

This membership offers:

It even covers treatment that could be excluded from some insurance plans.

This membership is valid for 1 year from the day you join. Make sure you take advantage of all cleanings, exams and treatment you need.

  • Sedation (Nitrous Oxide) - 100% (no charge)
  • Sealants - 50%
  • Oral Surgery (limited to extractions) - 15%
  • Fillings - 15%
  • Crowns - 15%
  • Night/Sports Guard - 15%
  • Dentures/Partials - 10%
  • Cosmetic Procedures - 10%
  • In Office Whitening - 10%
  • All other procedures - 15%

*Emergency Exam must be during regular business hours.
** Full Mouth Series/Panorex X-Rays every 3 years

Exclusions and Limitations

  • This membership plan is not an insurance plan. It may not be used with any dental insurance or another dental plan.
  • It cannot be used for services covered under workman's compensation.
  • It does not apply to referrals or hospitalization or hospital charges of any kind.
  • This membership may not be combined with any other offers, discounts, insurance or Care Credit.
  • All payments are NON-REFUNDABLE and is the responsibility of the member to utilize the plan.
  • Any missed confirmed appointments will incur a $50.00 non-refundable charge.
